How to Create a Provider Account at Sofia Health

Welcome to Sofia Health! To join as a provider, please follow the steps below:

1. Go to sofiahealth.com and on the top right corner select Join. 
2. Select Set Up Your Business.
3. Fill out the form to register. Use your email address to create an account, and select Continue.
4. Read through Sofia's Terms & Conditions, then type ‘I Agree’ in the box at the end.
5. Tap Sign up.
6. You will be prompted to choose how you would like to use Sofia Health for your business. Select one of the following options:
   1. List services
   2. List services and sell products
   3. Sell products only
7. Then you will be directed to the 'Free Trial & Payment Information' page.
   1. If you have a promo code, enter it in the designated field to take advantage of any special offers.
   2. Confirm your monthly subscription by selecting the radio button.
8. Click the 'Add Payment Information' button to continue.
9. Fill in your credit card information as required. Click ‘Start trial’ to complete the process.

Additional Information

• If you decide not to continue, make sure to cancel your subscription before the free trial ends to avoid any charges.
